Step 1: Download and complete the following forms. Please read the entirety of the documents and sign/fill out each section.
Step 2: Please complete our patient survey and a member of our clinic team will contact you after reviewing the form to begin the intake process.
Step 3: A member of our clinic will reach out to schedule an initial appointment and to provide further instructions. Due to the high volume of need at this time, wait times for receiving a call back for scheduling are higher than normal and can be between 2–3 weeks. Additionally, neuropsychological assessments are now being scheduled 2–3 months out. Thank you for your patience as we work to manage the high volume of requests.

RUSH MYCHART:
We also encourage our patients to enroll in the Rush MyChart system in order to receive appointment reminders, review billing information, and access the entirety of the Rush system. For more information and for help setting up the MyChart system, please visit https://mychart.rush.edu/
